1. U.S. and Global Approaches to Financing Long-Term Care: Understanding the Patchwork

Abstract:: Most United States adults age 65 and older will need long-term care toward the end of their lives.1 Long-term care, provided in the home, community, or in institutional settings like a nursing home, includes medical care as well as personal, nonmedical care for people who are unable to perform activities of daily living, like cooking or bathing. Some services also may support social needs for those unable to perform what are known as instrumental activities of daily living, such as shopping or medication management. In 2018, more than 5 million people in the United States used home health care, including adult day care or home health agency services, and nearly 4 million people resided in nursing homes or residential care communities.2 Despite the widespread need for long-term care, the U.S. has no national program to cover the costs of long-term care. While certain states have implemented their own programs, the resulting patchwork of funding has left many Americans with no sustainable way to pay for the services they require. Other developed countries have comprehensive, national long-term care financing systems featuring broad eligibility and high levels of financial support. In this brief, we analyze data from multiple sources on long-term care costs in the U.S., as well as spending data from the Organisation for Economic Co-operation and Development (OECD), to better understand long-term care financing strategies in 12 high-income countries. In addition to these data, we synthesized initiatives to long-term care funding in these 12 countries. (For details, see “How We Conducted This Study.”) Since the U.S. does not have a national funding strategy, we also focused on individual state approaches. International and U.S. state approaches may provide insights to policymakers seeking to advance comprehensive long-term care funding.

Abstract:: Climate-related loss and damage is escalating, with many countries experiencing new forms of climate impact, of increasing intensity. The focus until now has been on calculating the economic repercussions of climate risk. Its wider impacts and resulting hidden costs of climate change — such as loss of cultural heritage — are less understood and harder to quantify. This knowledge gap on non-economic loss and damage, which is largely due to limited coordination around research and sparse evidence originating from the global South, urgently needs addressing. This briefing proposes a comprehensive approach for building the evidence base on non-economic loss and damage, particularly the creation of a Loss and Damage Research Observatory. This online platform will lay the foundations for a collaborative South–South–North community of practitioners, ultimately leading to informed policy on this critical area of climate action.

3. The UN’s Response to the COVID-19 Infodemic

Abstract:: The COVID-19 pandemic has been accompanied by a related “infodemic.” This infodemic emerged as widespread demand for information about the public health emergency was met with large volumes of false and misleading information. Many of the national and international institutions leading the response to the COVID-19 pandemic, including the UN, quickly recognized that they also needed to respond to the infodemic. This led to a quick, broad-based response by a broad array of UN agencies and partners. To date, however, there has been no systematic mapping or assessment of the range of initiatives across the UN system. To begin filling that gap, this paper provides an overview of the UN system’s response to the COVID-19 infodemic across four areas: monitoring harmful information related to COVID-19; dispelling false information and providing authoritative information; building information resilience; and setting the agenda. It then assesses some of the successes and challenges of the response across four areas: external partnerships, including with governments, civil society, academia, and social media companies; coordination within the UN system; financial, human, and technological capacity; and impact assessment. The report concludes that the UN’s infodemic response finds itself at a pivotal point: Due to donor fatigue around COVID-19, funding for the infodemic response is falling even while the need for a robust infrastructure to address infodemics and other information disorders may be increasing. UN entities working to address the infodemic and other information disorders should therefore consider the following: Engaging in a consultative process to develop a shared understanding of the infodemic by defining its specific harms in relation to each of their mandates; Sustaining and building capacity to counter infodemics and other information disorders, both in communications departments and in other sections working on infodemic management; Adopting a more standardized, system-wide approach to the use of new technologies and engagement with technology platforms; and Continuing to strengthen long-term information resilience by building the capacity of governments to manage infodemics and supporting strong, independent media.

4. Disease Mix and How Economic Freedom Matters for Health Outcomes

Abstract:: We investigate the institutional foundations of public health. We argue that a key distinction in analysis of disease is between diseases of commerce (diseases associated with movement of people and with affluence) and diseases of poverty (primarily noncommunicable diseases that depend on wealth and income). We show that the mix of disease – the ratio of communicable diseases and those associated with longevity to diseases of poverty – increases in economically free countries. We argue that increasing burdens of diseases of commerce reflects the quality of institutions, as those diseases are better than living shorter, brutish lives where diseases of poverty claim many lives. This analysis also highlights an institutional trade‐off: economically free institutions reduce certain types of disease while contributing to others.

Abstract:: In this study, we aimed to examine the interconnectedness of health and peace, recognizing its significance within global health diplomacy, international relations, and human rights. For that purpose, we used the results from previous and ongoing Global Burden of Disease studies, which represent a comprehensive systematic appraisal of health problems and risks affecting populations worldwide. This paper could use its methodological underpinnings to analyze the impact of war, conflict, and terrorism on mortality and overall human health. In 2000, war and conflict were responsible for an estimated 310,000 deaths globally, compared to 2019, when this number decreased to 69,000. Recent findings reinforced the association between war, conflict, and increased all-cause mortality. Interpersonal violence also significantly contributed to human health loss resulting from disrupted peace. In Europe, disability-adjusted life years due to injury – including those caused by conflict – declined between 2000 and 2019. As we prioritize global health, peace-building initiatives, and global health diplomacy, big data will increasingly play a substantial role in accurately predicting and describing the health effects related to conflicts.

6. Five Urgent Questions on Ecological Security

Abstract:: The increasing pressure of ecological disruption on people and on security means that ideas and policy on peace and security must increasingly address the need for ecological security. This paper poses five research questions concerning: (a) amplification of antimicrobial resistance (pathogens that are increasingly drug-resistant); (b) the physiological consequences of pollution; (c) the loss of nature’s contribution to people’s well-being; (d) local and regional ecological tipping points; and (e) detrimental organisms and processes that thrive in the rapidly changing planet. Each question has a human health dimension, with likely socio-economic impacts and effects on behaviour, as well as potential effects on security and political stability. Understanding these issues is essential if appropriate responses are to be developed. More research is needed in both the natural and the social sciences, with interdisciplinary work that is in close contact with the policy world. The situation is urgent and policy responses cannot wait until all the answers are known and uncertainty has been fully eliminated.

7. Planetary health: A holistic vision for people and the planet

Abstract:: The nexus between health and the environment has been underscored by crises such as COVID-19 and the climate crisis. As awareness of policy interlinkages grows, so does the need for more coordinated EU and national policymaking. Calls for a more holistic approach have long been recognised, but the pandemic has further emphasised the importance of dismantling existing barriers in policymaking to improve the health status of people, animals, and the planet. This Discussion Paper, funded by Sitra, the Finnish Innovation Fund, advocates for implementing a planetary health approach across all levels of governance to encourage transdisciplinary action and puts forward ten recommendations: Pursue coordinated policymaking by breaking down silos. Appoint an Executive Vice President for the Well-being Economy. Plan and implement national roadmaps and establish common indicators. Create best practices platforms. Adopt planetary health for health systems. Promote education on planetary health. Adopt a European-wide communication campaign. Invest in research and innovation. Foster the integration of the European Health Data Space with future data spaces. Work with international partners to adopt a global planetary health approach. Planetary health offers many benefits for health and the environment. However, without concentrated efforts to move beyond the traditional siloed approach to policymaking, the benefits may never be realised.

Abstract:: The COVID-19 pandemic has claimed nearly one million American lives. It has upended many more and produced disastrous consequences around the world. But it could have been even more catastrophic had Western nations and companies not developed highly effective vaccines. As the world hopefully begins to emerge from the pandemic, it is important to reassess the global biological threat landscape, including the possibility that bioterrorists or other bad actors might seek to exploit advances in biotechnology to engineer a future pandemic. In a joint effort, the Combating Terrorism Center and the Department of Chemistry and Life Science at West Point have assembled some of the best and brightest thinkers in the counterterrorism, policy, and scientific communities around the world for their perspectives and analysis on the evolution of the biological threat picture. The result is a two-volume set of special issues, with the second volume being published next month. The feature article of this volume anticipates how bioterrorists may exploit tunable viral agents in a new age of bioterrorism. The article is authored by Major Stephen Hummel and Colonel F. John Burpo with Lieutenant Colonel Jeremy Hershfield, Lieutenant Colonel Andrew Kick, Kevin O’Donovan, and Colonel Jason Barnhill. They write: “Components of a disease such as transmissibility, lethality, and the infectious window can potentially be modified for desired tactical, operational, or strategic effects. While this capability is currently beyond the skills and knowledge of the biology enthusiast, a trained individual would possess such skills and knowledge, though they may lack the necessary material and infrastructure support. Hence, it is necessary to develop and maintain capabilities that can respond to a variety of pathogens and possible effects.” This issue features an interview with Lawrence Kerr, who until his retirement from government service earlier this year was the Director of the Office of Pandemics and Emerging Threats in the Office of Global Affairs at the U.S. Department of Health and Human Services. We also interview Brad Ringeisen, the Executive Director of the Innovative Genomics Institute. Before his retirement from government service in July 2020, Dr. Ringeisen served as the Director of the Biological Technologies Office at the Defense Advanced Research Projects Agency (DARPA). Filippa Lentzos, Gregory Koblentz, and Joseph Rodgers argue “the biological risk landscape is rapidly evolving and presents significant new challenges to preventing the accidental, reckless, or malicious misuse of biology. At the same time, oversight systems to ensure that life sciences research is conducted safely, securely, and responsibly are falling behind. An urgent overhaul to realign biorisk management with contemporary risks is needed.”

9. The Next Game Changers: A Priority Innovation Agenda for Global Health

Abstract:: Over the last century, scientific and technological innovation has led to unprecedented improvements in health outcomes—yet research and development (R&D) investments and progress to address health threats has been uneven. Commercial R&D has focused where investors can expect substantial financial returns: rich countries, the diseases that affect them, and high-tech solutions designed for the richest and most sophisticated systems. Despite supplemental funding from philanthropic and government grants, R&D to address many leading causes of death and disability—especially those that primarily affect low- and middle-income countries (LMICs) or insure against future risk—has been consistently underfunded relative to potential health gain. This implies that many untapped opportunities remain to dramatically improve global health and welfare via biomedical innovation. In this paper, we report the results of a horizon-scanning exercise to source opportunities for global health R&D investment—that is, high-value potential biomedical innovations which are currently underfunded but which could be transformative for health, quality of life, and health security in LMICs and around the world. Drawing from a literature review and expert interviews with researchers, economists, funders, advocates, and implementers, we lay out an expansive and high-promise (though non-comprehensive) biomedical innovation agenda for global health spanning the unfinished MDG agenda; non-communicable diseases; and global health security. We conclude with a discussion of implications for research, funding, and practice.

10. A Fund for Global Health Security and Pandemic Preparedness

Abstract:: The COVID-19 crisis has served as a painful demonstration that no country is fully prepared for a pandemic and that the existing global health architecture remains ill-equipped to finance pandemic prevention, preparedness, and response (PPR). The rationale for preventing and containing pandemics is self-evident: the price of preparedness is a fraction of the cost of responding to catastrophic outbreaks, both in terms of human and economic well-being.[1] In the aftermath of the 2003 SARS outbreak, 2009 H1N1 pandemic, and the 2014-2016 West Africa Ebola outbreak, experts called for scaling up sustainable financing to ensure the world was better prepared to face that next global health threat. But even amid efforts to better understand and measure preparedness gaps,[2] little action was taken to mobilize financing and incentivize investments in strengthening detection, prevention, and response capabilities. As global health threats evolve, countries’ capacity to prepare for and respond to disease outbreaks is increasingly a global imperative. Now is the time to take concrete steps toward establishing sustained financing for pandemic PPR to help bring an end to the ongoing COVID-19 pandemic, combat future health security threats, and break the costly cycle of neglect.

11. Prioritizing Public Spending on Health in Lower-Income Countries: The Role of the Global Financing Facility for Women, Children and Adolescents

Abstract:: The Global Financing Facility (GFF), launched in 2015, is a partnership backed by a multi-donor World Bank trust fund that aims to “prioritize and scale-up evidence-driven investments to improve reproductive, maternal, newborn, child, and adolescent health and nutrition (RMNCAH-N) interventions” in low- and lower middle-income countries. A major component of the GFF’s operational model is centered around the investment case, described by the GFF as “a description of the changes that a country wants to see with regard to reproductive, maternal, newborn, child, and adolescent health (RMNCAH) and a prioritized set of investments required to achieve these results.” Looking retrospectively at investment cases and associated ongoing World Bank operations, this paper aims to shed light on specific aspects of the GFF’s operating model, approach, theory of change, and results to date related to the “development of a costed and prioritized investment case,” and whether the investment case helps to prioritize public spending on health in the real world. The portfolio-wide findings suggest that the GFF delivers a clear value-add within the global health architecture by directing more funding to flow directly through governments via increased World Bank lending for RMNCAH-N (especially in the context of COVID-19-related service disruptions and fiscal contractions). The role of the investment case in aligning and prioritizing resources behind a consensus set of highest-value RMNCAH-N interventions within a specific funding envelope is less well documented in the public domain; GFF efforts to directly influence government RMNCAH-N spending decisions and their efficiency seem to occur primarily through follow-on resource mapping and expenditure tracking. The paper concludes with three recommendations for the GFF and its donors: (1) pursue a deliberate strategy for prioritization of RMNCAH-N services and products within the realistic budget constraint to advance evidence-based decisions on certain trade-offs; (2) increase transparency and public visibility into GFF-supported activities and funding, especially related to RMNCAH-N product procurement and overall resource tracking; and (3) explore incentives for increased donor co-financing within the framework of World Bank projects as a strategy to pool and channel more resources through government coffers.

Abstract:: The wealth of the world’s 10 richest men has doubled since the pandemic began. The incomes of 99% of humanity are worse off because of COVID-19. Widening economic, gender, and racial inequalities—as well as the inequality that exists between countries—are tearing our world apart. This is not by chance, but choice: “economic violence” is perpetrated when structural policy choices are made for the richest and most powerful people. This causes direct harm to us all, and to the poorest people, women and girls, and racialized groups most. Inequality contributes to the death of at least one person every four seconds. But we can radically redesign our economies to be centered on equality. We can claw back extreme wealth through progressive taxation; invest in powerful, proven inequality-busting public measures; and boldly shift power in the economy and society. If we are courageous, and listen to the movements demanding change, we can create an economy in which nobody lives in poverty, nor with unimaginable billionaire wealth—in which inequality no longer kills.

13. Vaccine Equity in ConflictAffected Areas: The Challenges of Development, Production, Procurement, and Distribution

Abstract:: While the wide-ranging impacts of COVID-19 are being felt in all countries and communities, the pandemic is having a disproportionately large impact on vulnerable populations, such as people living in areas affected by fragility, conflict, and violence. Vaccines hold enormous promise to mitigate these impacts, but the complications inherent to armed conflict make accessing vaccines especially challenging. Even when countries do receive vaccines, they often face challenges in rolling out vaccination programs, whether because they lack adequate capacity or because the doses are set to expire or are not acceptable to the communities set to receive them. This issue brief focuses on the challenges of equitably distributing COVID-19 vaccines to populations in conflict-affected areas. It begins by looking at general issues related to the development, approval, production, procurement, and distribution of vaccines. It then examines the particular challenges to distributing vaccines in conflict-affected areas both before and during the vaccine rollout. The report concludes with several recommendations for stakeholders and policymakers to improve the delivery of vaccines in conflict-affected areas: Redistribute global resources to increase the supply of vaccines to conflict-affected countries; Increase the transparency and predictability of global vaccine supplies; Enhance cooperation and coordination at the national and local levels to deliver vaccines to conflict-affected areas through existing humanitarian response mechanisms; and Ensure that vaccination campaigns in conflict-affected areas adhere to humanitarian principles.

14. Heroes on Strike: Trends in Global Health Worker Protests During COVID-19

Abstract:: This Accountability Note describes global trends in health worker protests between March 11, 2020 and March 10, 2021. We argue that the frequent characterization of health workers as heroes of the pandemic obscures the fact that health workers themselves describe the challenges they face during this time as violations of their rights as workers, highlighting the obligations of their employers to provide working conditions and remuneration that justify the risk they assume. Using data from the Armed Conflict Location & Event Data (ACLED) Project, we first provide a global overview of the growth in health worker protest activity in the March 2020–2021 period as compared to the March 2019–2020 period. Health workers engaged in protest activity with greater frequency during the first year of the pandemic than during the prior year. The eighty-five countries included in the ACLED data set in both the March 2019–2020 period and the March 2020–2021 period experienced a sixty-two percent increase in health worker protest activity (from 2416 protests to 3913 protests). We then provide an overview of the content of the 6589 health worker protests in the ACLED dataset between March 2020 and March 2021, describing protests in 149 countries in terms of five categories: working conditions and remuneration, resources, health system delivery issues, public policy, and other. By far, the largest category of protests during this period are those related to working conditions and remuneration. Approximately sixty-six percent (N=4358) of global protests in the 2020–2021 period expressed health worker dissatisfaction about issues such as: occupational hazards, unpaid wages, risk allowances and job security. Surprisingly, personal protective equipment (PPE) concerns—an issue covered frequently in the media—motivated a relatively low percentage of the protests (less than nine percent). We note this not to downplay the gravity of global PPE shortages, but to situate PPE shortages as one among many health worker concerns during the pandemic. Protests initiated by health workers about health system delivery issues provide an important reminder that even in the middle of a pandemic, not everything was about COVID-19. Sixty-two percent of the health system-related protests during the 2020–2021 period were about issues that were not coded as directly related to the COVID-19 pandemic. Similarly, the vast majority of health workers’ protests around public policy were not about pandemic-related government policy, but about social issues like climate change, immigration, police brutality, and elections. The aim of this analysis is to understand what health workers advocated for during a time of immense pressure, and to identify some broad areas of concern for health sector accountability. Through this analysis, we prepare the groundwork for future research and action, and share preliminary guidance regarding how health sector stakeholders—including government, health worker associations, unions, and civil society—might develop improved systems of accountability.

Abstract:: This paper quantitatively describes different aspects of the COVID-19 pandemics: new cases, hospitalisations, intensive-care admissions and deaths, while illustrating their changing relationships over time. It then assesses how the different variables have affected relevant sectoral and macroeconomic indicators. Finally, it concludes that, from an economic perspective, what matters when it comes to managing the pandemic is to prevent intensive-care admissions and deaths arising from COVID-19. The success of vaccination should be measured in terms of its ability to prevent the most serious consequences, rather than its ability to prevent infections and hospitalisations.

The 2015 Paris Agreement’s target of restricting the average temperature rise in the 21st century to well below two degrees Celsius compared to pre-industrial levels is increasing unlikely, given the inaction of states and the global lack of progress in meeting Paris Agreement goals. As nations scramble to meet Nationally Determined Contributions (NDCs) and effectively design climate policies, the global crisis has given rise to a new domain, Planetary Health. Planetary Health, an interdisciplinary field, is described as a solutions-focused discipline which aims to analyse, mitigate, and resolve human disruption to Earth’s natural systems in the new Anthropocene epoch. The Anthropocene epoch, on the other hand, is established as a geological climate where human activity is drastically altering earth’s systems to an extent that will be reflected in fossil records. Planetary Health inextricably links human health to the health of the planet and illustrates the interdependency of the two, and this relationship has been substantiated by findings which concur that countries with higher ecological threats concurrently experience greater social vulnerability. When climate-induced disasters occur due to human action, there are subsequent consequences on food, water, energy, and human security. The consequences are disproportionately felt by vulnerable communities in states that lack resources and capabilities to address the consequent impacts on human security, and climate instability is a driving force behind political instability. Thus, there is cause for concern not only for the planet’s health but also for human health as the environment’s health continues to dwindle in the Anthropocene Epoch. As such, the concept of Planetary Health is a response to threatened climate security and human security.

Abstract:: More than two years into the COVID-19 pandemic, rising vaccination levels and widespread testing in many countries are giving governments and the public increasing confidence in international travel. A central part of efforts to restart mobility are digital health credentials, which verify a person’s vaccination, testing, and/or recovery status. These credentials help to minimize the risk of travelers carrying the virus, and unlike paper credentials that must be manually inspected in airports and at borders, digital credentials can be automatically verified. But even as digital health credentials are becoming increasingly common, gaps in international coordination and technical integration issues between different credential systems remain. As a result, migrants and travelers moving between—and at times within—countries must navigate a complex maze of digital systems. This report examines the implications of digital health credentials for international travel and for immigrants’ and travelers’ access to domestic services and venues that also use such credentials. It also explores the implications for specific groups: tourists and business travelers, students, labor migrants, asylum seekers and refugees, and unauthorized migrants. Finally, it offers policy recommendations to facilitate mobility and minimize risks for people on the move.

Abstract:: Attacks on healthcare in situations of armed conflict have been reported at alarming levels over the past two decades. In response to this problem, the UN Security Council unanimously passed Resolution 2286, which urges states to collect data on attacks on medical personnel, transport, and facilities. This data is essential to understand the scale and scope of the problem, protect health services and workers, prioritize resources to those most impacted, prevent future attacks, and hold perpetrators accountable. This paper examines why data on threats to and attacks on healthcare in conflict is important to protection, advocacy, and investigation and how it can be improved and harmonized. It provides an overview of existing data-collection efforts—namely, the Surveillance System for Attacks on Health Care (SSA) and the database produced by the Safeguarding Health in Conflict Coalition (SHCC) in partnership with Insecurity Insight—and identifies challenges and gaps at both the policy and technical levels. This paper concludes with the following recommendations for the World Health Organization (WHO), other UN entities, UN member states, and NGOs: The World Health Assembly should adopt a resolution calling on WHO to address the major concerns in the structure and operation of the SSA; WHO should make technical improvements to the quality and presentation of data in the SSA and be open to a range of data-collection methodologies; Other UN agencies, governments, and civil society organizations should take steps to improve the collection and sharing of data on attacks on healthcare to improve protection, prevention, and accountability; and Governments, NGOs, and other actors should increase the funding and capacity of existing data-collection initiatives.

22. A lockdown a day keeps the doctor away: The effectiveness of non-pharmaceutical interventions during the Covid-19 pandemic

Abstract:: Countries have employed a variety of non-pharmaceutical interventions (NPIs) in order to curtail the COVID-19 pandemic. However, the success of individual measures in reducing the number of infections remains controversial. This paper exploits a panel data set of 107 countries to estimate the effects of 14 NPIs on the spread of the disease. While almost all measures had a dampening effect on the reproduction rate of the virus, public information campaigns and school closings were most effective, followed by testing policies, contact tracing and international travel restrictions. Public event cancellation and school closings were less effective during the second wave of the pandemic, while public information campaigns and the obligation to wear masks worked better. Several NPIs had a stronger impact on infections in autocratic countries, while others were less effective.

23. Where are the Global COVID-19 Resources for LGBTI Communities?

Abstract:: As COVID-19 spread across the globe in 2020, and its health and broader political and socioeconomic implications became evident, lesbian, gay, bisexual, transgender, and intersex (LGBTI)2 communities organized. To meet new challenges, LGBTI organizations across the world stepped up, aware that legal and social discrimination and marginalization would make their communities particularly vulnerable to impacts of the pandemic. LGBTI community response included: delivering essential food to communities of unemployed trans men in rural Guatemala; providing housing for LGBTI communities escaping unsafe living environments in Macedonia; ensuring that lesbian, bisexual, and queer female sex workers have access to essential medicines in Uganda; and other examples in communities around the world. As governments, donors, and service providers have largely failed to acknowledge the specific needs of LGBTI people in responding to COVID-19, LGBTI organizations have filled the void to provide basic protection and support for their communities. Many of these organizations have traditionally focused on advocacy and community organizing to advance and protect the human rights of LGBTI people. Now, in the era of COVID-19, they have become direct service providers, out of necessity—albeit with limited resources and capacity. In April 2020, the Global Philanthropy Project launched a short survey to understand the initial response of global LGBTI philanthropy to the pandemic, soliciting data from all GPP member organizations as well as non-GPP members within the top 20 funders of global LGBTI issues. A key outcome from that report was an identified role for GPP to monitor shifts in resources flowing to LGBTI movements and communities, as well as the broader impact of COVID-19 on international development and humanitarian assistance funding.

Abstract:: During the Covid-19 pandemic, Europe has benefitted strongly from being an open economy that can access goods and services from other parts of the world. Paradoxically, some politicians in Europe think that dependence on foreign supplies reduced the resilience of our economy – and argue that Europe now should wean itself off its dependence on other economies. In this Policy Brief, it is argued that self-sufficiency or less economic openness is a dangerous direction of policy. It would make Europe less resilient and less capable of responding to the next emergency. It is key that people, firms and governments can get supplies from other parts of the world. It is diversification, not concentration of production, that will make Europe more resilient when the next emergency hit. We don’t know where the next crisis will come from. Nature will throw nasty surprises at us, and we will make stupid mistakes, some of which will have devastating consequences. What we do know, though, is that we stand a better chance to fight the next emergency if we get richer and improve our technology. The best policy for resilience is one that encourages specialisation and innovation – and, when the emergency hit, allow for people to improvise in search for solutions. For that to happen, we need openness to goods, services and technology from abroad.

Abstract:: If 2020 was the year of “mask diplomacy”, as countries raced to tackle the spread of the SARS-CoV-2 virus and acquire the necessary protective gear and equipment, 2021 is likely to be remembered as the year of “vaccine diplomacy”. Growing competition between states to secure the necessary quantities of vaccines to inoculate their population has already become an established feature of the post-COVID international system and such trends are only likely to increase in the near future. It normally takes up to a decade to transition from the development and testing of a vaccine in a laboratory to its large-scale global distribution. Despite current challenges, the speed of COVID-19 vaccination campaigns is unprecedented. To put an end to the current pandemic – which in one year has led to the loss of 2.6 million lives and triggered the worst economic recession since the Second World War – the goal is to ensure the widest immunisation of the world population in a timeframe of 12 to 18 months. In this context, COVID vaccines emerge as instruments of soft power, as they symbolise, on the one hand, scientific and technological supremacy and, on the other, means to support existing and emerging foreign policy partnerships and alliances with relevant geopolitical implications. From their experimentation in laboratories, to their purchase and distribution, the vaccine has emerged as a significant tool for competition between powers, often associated with the promotion of competing developmental and governance models across third countries.

27. Meeting the Global Health Challenge to Reduce Death and Disability from Alcohol, Tobacco, and Sugar-Sweetened Beverage Consumption with Corrective Taxes

Abstract:: Behind the daily trauma of COVID-19 lies a larger and longer-lasting global health challenge resulting from the consumption of tobacco, alcohol, and sugar-sweetened beverages. Using a sample of 25 large advanced and emerging market economies accounting for three-quarters of global GDP, we show that about 60 million productive life years were lost every year in 2000, 2010, and 2019 from death and disability attributable to alcohol, tobacco, and diets high in sugar-sweetened beverages. Using the cost-of-illness framework, the economic value of these lost productive years in 2019 is approximately $2.1 trillion in our country sample (in 2017 purchasing power parity dollars [PPP$]). We show that over a 20-year period, a higher tax effort is associated with larger reductions in economic costs. We also show that total corrective taxes for all the products under investigation fall far short of the indirect costs of consumption in all countries. We conclude that all advanced economies and emerging markets could reap substantial macroeconomic benefits from better health by raising corrective taxes on alcohol and tobacco and by introducing corrective taxes on sugar-sweetened beverages.

Abstract:: About one-quarter of U.S. adults report having a mental health diagnosis such as anxiety or depression or experiencing emotional distress. This is one of the highest rates among 11 high-income countries. While U.S. adults are among the most willing to seek professional help for emotional distress, they are among the most likely to report access or affordability issues. Emotional distress is associated with social and economic needs in all countries. Nearly half of U.S. adults who experience emotional distress report such worries, a higher share than seen in other countries. The United States has some of the worst mental health–related outcomes, including the highest suicide rate and second-highest drug-related death rate. The U.S. has a relatively low supply of mental health workers, particularly psychologists and psychiatrists. Just one-third of U.S. primary care practices have mental health professionals on their team, compared to more than 90 percent in the Netherlands and Sweden.

Indeed, added restrictions have been imposed on medical trade during the pandemic. Meanwhile, governments have accomplished little at the World Trade Organization (WTO) to help meet this need. Using trade to help fight the COVID-19 pandemic and to otherwise support global health must move to the top of the WTO agenda, with the aim of finalizing new rules to support trading for health care goods by the time of the next WTO ministerial conference in Geneva in late November 2021 — and ideally, sooner.

Abstract:: The COVID-19 pandemic has sparked controversies over health security strategies adopted in different countries. The urge to curb the spread of the virus has supported policies to restrict mobility and to build up state surveillance, which might induce authoritarian forms of government. In this context, the Copenhagen School has offered an analytical repertoire that informs many analyses in the fields of critical security studies and global health. Accordingly, the securitisation of COVID-19 might be necessary to deal with the crisis, but it risks unfolding discriminatory practices and undemocratic regimes, with potentially enduring effects. In this article, we look into controversies over pandemic-control strategies to discuss the political and analytical limitations of securitisation theory. On the one hand, we demonstrate that the focus on moments of rupture and exception conceals security practices that unfold in ongoing institutional disputes and over the construction of legitimate knowledge about public health. On the other hand, we point out that securitisation theory hinders a genealogy of modern apparatuses of control and neglects violent forms of government which are manifested not in major disruptive acts, but in the everyday dynamics of unequal societies. We conclude by suggesting that an analysis of the bureaucratic disputes and scientific controversies that constitute health security knowledges and practices enables critical approaches to engage with the multiple – and, at times, mundane – processes in which (in)security is produced, circulated, and contested.

While states around the world are redeploying their financial resources, energies and military capabilities to cope with the challenge of the coronavirus, terrorist groups across the ideological spectrum have positioned themselves to exploit the gaps created by these policy re-adjustments. Terrorist groups are milking people’s fears amid confusion and uncertainty to promote their extremist propagandas. The rearrangement of global imperatives will push counter-terrorism and extremism down the priority list of the international community. Anticipating these policy changes, existing counter-terrorism frameworks and alliances should be revisited to devise cost-effective and innovative strategies to ensure continuity of the fight against terrorist groups. With these considerations in mind, this special issue of the Counter Terrorist Trends and Analyses (CTTA) features four articles that identify and assess important security risks around COVID-19, given its far-reaching social, economic and geopolitical impact. In the first article, Raffaello Pantucci reasons that COVID-19 will have a deep-seated and prolonged impact across government activity, both in terms of the categorisation of risks, as well as the resources available to tackle other issues. Perceptions of risk around terrorist threats may shift, with states grappling with stark economic, social and political challenges. At the same time, security threats continue to evolve, and may even worsen. According to the author, some of the tools developed to deal with the pandemic can potentially be useful in tracking terrorist threats. However, resource constraints will require states, on a global scale, to think far more dynamically about how to adequately buffer much-needed security blankets both within and beyond their borders. In the second article, Abdul Basit outlines the opportunities and potential implications that COVID-19 has created for terrorist groups across the ideological divide. According to the author, terrorist groups have exploited the virus outbreak to spread racial hatred, doomsday and end-of-times narratives. Among jihadist groups, IS has taken a more totalitarian view of the coronavirus pandemic, while Al-Qaeda (AQ) and the Taliban have used it as a PR exercise to gain political legitimacy. Far-right groups in the West have spun it to promote native nationalism, border restoration and anti-immigration policies. Terrorist groups have increased their social media propaganda to radicalise and recruit vulnerable individuals. At the same time, these groups have urged their supporters to carry out lone-wolf attacks and use the coronavirus as a bioweapon. In the post-COVID-19 world, revisiting existing counter-terrorism frameworks to devise more adaptable and cost-effective strategies would be needed to continue the fight against terrorism. In the next article, V. Arianti and Muh Taufiqurrohman observe that the COVID-19 outbreak has had a varied impact on Indonesia’s security landscape. On the one hand, it has emboldened IS-affiliated Indonesian militant groups to step up calls for attacks, with the government seen as weakened amidst a worsening domestic health crisis. On the other, ongoing indoctrination and recruitment activities of militant groups have also faced disruptions. According to the authors, counter-terrorism strategies will need to be reoriented as circumstances evolve, particularly in dealing with the arrest of militants and the subsequent processes of their prosecution and incarceration. Finally, Kyler Ong and Nur Aziemah Azman examine the calls to action by far-right extremists and the Islamic State (IS), which reveals varying degrees of organisational coherence in the respective movements. According to the authors, such variations influence these two groups’ preferred techniques, tactics and procedures adopted in seeking to exploit the health crisis. For its part, IS has a more organised hierarchical structure, even if it has increasingly granted autonomy to its affiliates to plan and execute attacks. In comparison, the absence of a central authority, or command structure in the far-right, can lead to a fragmentation of interests. These factors invariably create uncertainties in how, when and where extremists of both ilk may seek to operationalise an attack.

35. The Coronavirus Crisis: Origins and the Way Forward

Abstract:: EXECUTIVE SUMMARY: Amid the debate on the coronavirus crisis, there is broad agreement on three issues: The nation-state has failed to check the spread of the virus, quickly and with few people being infected, by using its autonomous capabilities, which turned out to be meager. Trans-state bodies that derived their economic capabilities from the state have failed in their role of assisting it. The idea of globalism is fundamentally true, and the problems that have emerged in the crisis must be remedied by strengthening the states and, at the same time, as concluded by French President Emmanuel Macron, the trans-state bodies. This study contends that globalism in its current form has failed and collapsed, just as communism and other social frameworks failed and collapsed before it. The reason for their collapse was that all of them were based on delusory utopian ideas. These utopian ideas are grounded in a dominant European liberal discipline whose founders abandoned the scientific revolution at the beginning of its path, abjured rational thought, and continued, like the church, to adhere to faith-based thought, feelings of the heart, and delusions of the imagination. Another liberal discipline, less well-known, is the rational one. Committed to truth and to the absolute laws of nature, it was adopted by the Anglo-Saxon democracies, which, thanks to its values, experienced long periods of growth and prosperity. On three occasions this discipline could be mobilized to help rescue Europe from calamities that its utopian conceptions had caused.

Abstract:: The world's leading economic powers must cooperate more to combat the health and economic shocks resulting from the COVID-19 pandemic. In a new PIIE Briefing, Peterson Institute experts outline how collective action by the Group of Twenty (G20) nations can make a difference. The PIIE agenda includes removal of trade barriers impeding the flow of medical supplies and food, and more money for research, testing, and disease control, especially for debt-burdened low-income countries. The World Bank and the World Health Organization need more resources to relieve suffering, and the International Monetary Fund must step up to stabilize the world financial system.

Abstract:: In April 2020, GPP conducted a survey in order to understand emerging practices and principles for COVID-19 response among funders supporting global LGBTI issues, share common learnings and opportunities for collaboration, and inform how GPP can support the community of global LGBTI funders over the course of the pandemic and beyond. The survey focused on GPP member funds as well as non-GPP members in the top 20 funders of global LGBTI issues (based on the 2017-2018 Global Resources Report). The brief contains findings on: Common principles and innovative initiatives global LGBTI funders have commenced since the pandemic began; Key issues that LGBTI funders are hearing from grantee partners; Actions global LGBTI funders have taken to response to these issues; Initial planning or forecasting on implications for global LGBTI funders, including new resources; Recommendations for global LGBTI philanthropic community collaboration in response to COVID-19 impacts on LGBTI communities across the world. In response to the key recommendations from this survey, GPP will monitor shifts in resource flows to LGBTI movements and communities resulting from the COVID-19 pandemic. We plan to undertake a follow up survey of these leading global LGBTI funders in July 2020 and will also publish those results here.

Abstract:: The COVID-19 pandemic knows no borders. It further knows no gender, class, or race. This virus does not discriminate, but our societies do. Around the world we have historically built systems and structures that privilege the few and disadvantage the many. When a crisis as unprecedented as the current pandemic hits, inequalities are exacerbated. This holds particularly true for gender equality which, despite encouraging steps forward, no country is on track to achieve by 2030. This not only fails politically marginalised groups, in particular women, girls, and gender nonconforming people, but also greatly hinders the international community’s commitment to foster peace and security. Research shows that the most significant factor in determining a country’s peacefulness (within its borders and towards other countries) is its level of gender equality. Already in early April, the UN warned in its policy brief, “The Impact of COPVID-19 on Women”, that the limited “gains made in the past decades [towards gender equality] are at risk of being rolled back.” Governments and foreign ministries must apply a feminist perspective to their COVID-19 response in order to to prevent a set-back, safeguard existing progress, and advance more quickly toward their goals: A ‘gender-blind’ approach would counteract all previous efforts not only in the area of gender equality, but also in conflict prevention and the pursuit of international peace.

As the core global health governance institution, the WHO has taken a series of actions after the eruption of the epidemic and played an important role in providing guidance and coordinating global forces to combat the epidemic. The epidemic, however, has also exposed some defects of the global health governance mechanism, especially the WHO, and posed challenge to it. The international community needs to strengthen its support for the WHO to improve its role in the global health governance.

Abstract:: Developed by Brown University faculty with expertise in infectious diseases, emergency medicine, medical education, and instructional design, the training curriculum has been designed in accordance with World Health Organization standards, best practices, and guidelines. The training program, teaches and evaluates trainees on COVID-19 core competencies, including biology and transmission mechanisms; infection prevention and control; contact tracing; screening and triage; diagnosis and management; stabilization and resuscitation; health facility operations and surge capacity; and risk communication and public health messaging. Designed for ease of remote access, the training program is delivered virtually by Brown University trainers and is accompanied by real-time Q&A support. Tailored for each specific country setting in which it is delivered, the curriculum focuses on strengthening the capacity of health workers in resource-limited settings. The training utilizes a Training-of-Trainers model to engage master trainers and coach new trainers, allowing Project HOPE and CHRHS to quickly build a cadre of competent instructors who are able to train other health workers within their networks to respond efficiently to the threat of COVID-19 while also protecting their own health.

47. Is the Coronavirus Catalyzing New Civic Collaborations for Open Government?

Abstract:: The pandemic is spurring elite and grassroots civic actors to cooperate more, but the gulf between them remains wide. Civic actors must seize the opportunity for reform on open government issues. From Africa to Latin America to Europe, the coronavirus pandemic has generated a surge in public demand for government transparency and accountability. To seize this window for reform, elite and grassroots civic actors concerned with open governance must overcome the cleavage that has long existed between them. Thus far, the pandemic has catalyzed some new civic collaborations, but not at the scale or depth needed to seize that window. In general, civil society groups report feeling more isolated during the pandemic. In some places, the urgency of tackling open government issues during the pandemic has helped overcome that isolation by deepening partnerships among existing networks. But in other places, those partnerships have yet to take shape, and new alliances are less likely to form without the benefit of face-to-face interactions. Even the partnerships that have crystallized or deepened do not appear to be changing the fundamental roles of elite and grassroots civic actors. It is possible that this shift may happen over time. Or it may be that the pandemic alone is not enough to dislodge structural barriers to deeper cooperation. The pandemic has dramatically changed the operations of elite and grassroots actors alike. The impact of those changes on collaboration between the two depends on preexisting levels of technological capacity. In places with limited connectivity, the pandemic has exacerbated the digital divide, adversely affecting grassroots actors. Meanwhile, in places with good connectivity, technology is enabling broader (though shallower) participation, laying the groundwork for more elite-grassroots collaboration. Although many civil society groups are struggling financially during the pandemic, those effects are mitigated to some degree by continuing donor interest in the open government sector. This is encouraging, as coalition building requires dedicated, flexible resources. Finally, it is a more dangerous time to be working on open government issues in general, and grassroots actors bear disproportionate risks in doing so. This underscores the need for more vertical alliances to mitigate civic space threats.
