Abstract:
Power is a critical factor in several types of diplomacy. India-EU relations are a classic case of how changing geopolitics prompted diplomatic acumen. This scoping review assesses the changing relations of both partners as a manifestation of Smart Power. The dynamic relations from normative-based to pragmatic and inclusive interests based are evaluated. Their value-based relations are put under international relations theories. Their changing factors of cooperation are used to justify their smart diplomacy, where contemporary relations are less likely to be affected by multilateral interests. To solidify claims of smart power in their relations, the recent TRIPS waiver schism illuminated health diplomacy between the two regions. This health diplomacy discourse promotes smart power diplomacy between India and the EU, where new avenues of cooperation emerge despite pandemic disagreements. The article explores how hybrid power is better than soft and hard power in silos by systematically searching and selecting the existing knowledge in the contemporary context.

2. The Labor Market Effects of Disability Benefit Loss

Disability benefits are costly and tend to reduce labor supply. While spending can be contained by careful targeting, correcting past flaws in eligibility rules or assessment procedures may entail welfare costs. We study a major reform in Hungary that reassessed the health and working capacity of a large share of beneficiaries. Leveraging age and health cutoffs in the reassessment, we estimate employment responses to loss or reduction of benefits. We find that among those who left disability insurance due to the reform 58% were employed in the primary labor market, 6% participated in public works and 36% were out of work without benefits in the post-reform period. The consequences of leaving disability insurance sharply differed by pre-reform employment status. 81% of beneficiaries who had some employment in the pre-reform year worked, while only 33% of those without pre-reform employment did. The gains of the reform in activating beneficiaries were small and strongly driven by pre-reform employment status. This points to the importance of combining financial incentives with broader labor market programs that increase employability.

What are the public policy challenges confronting the UK? What plans do the parties have for addressing them? To what extent are these plans fit for purpose? As Parliament returns from its summer recess, Full Fact and UK in a Changing Europe have partnered to produce a series of evidence led, research-based assessments of the key issues that politicians will confront. To do this, we brought together a group of leading experts from respected institutions including the Health Foundation, the Institute for Fiscal Studies, and the Royal United Services Institute. We asked each of them to consider, based on the findings of their own research, the challenges the country faces in their area of expertise, the immediate and longer-term issues politicians will need to address, and the potential implications of any choices they might make. We selected the issues based on the Ipsos issues index, and added security, defence and foreign affairs, which did not figure amongst the public’s priorities in July of this year. The intention of the collection is to inform and not persuade. Each article in this collection is the responsibility of its author.

Abstract:
The decision by Conjuntura Austral to dedicate a special issue to the 25th anniversary of the establishment of the Community of Portuguese Language Countries (CPLP) is to be commended. The uniqueness of the organization justifies a timely review of a small sample of its activities to date. Created in 1996, the CPLP adopted three main vectors of activity:1) the promotion and dissemination of the Portuguese language; 2) cooperation in a wide array of domains (i.e., education, health, science and technology, defense, agriculture, public administration, communication, justice, public safety, culture, sport and social communication, sustainable development, etc.); and 3) political and diplomatic coordination, especially in multilateral forums. The organization also agreed to adhere to several fundamental political principles such as the respect of the sovereignty of each member country; non-interference in domestic issues; reciprocal treatment; peace, democracy, rule of law, human rights, and social justice; territorial integrity of each member state; and the commitment to the promotion of development and cooperation. In addition to a common language and shared cultural heritage, the uniqueness of the CPLP results from the union of nine non-contiguous member states into a privileged geocultural space – the lusosphere – dedicated to the active promotion of its three main goals across four continents. It is a privileged forum for a “pluricontinental dialogue” in Portuguese. In addition, each country contributes by bringing its history, its unique interpretation of the “lusosphere” as well as its regional context (i.e., membership in the European Union, Mercosur, Southern African Development Community, Economic Community of West African States, etc.) as well as economic opportunities to the group. By joining, member countries reinforce the group’s collective projection onto the world stage as well as the opportunity to expand each member country’s diplomatic footprint. While each member had different reasons to join (i.e., maximize politicaldiplomatic cooperation, language promotion, widen access to technical cooperation, etc.), all believe that the CPLP can bolster the group’s overall political and diplomatic prestige while enhancing their collective and individual prospects. This special issue consists of six articles ranging from language to health, security and defense issues, business negotiations and civilian-military relations. It covers but a few topics from a long list of issues which, after the first twentyfive years of this new international organization, warrant the attention and critical review of academic scholars.

COVID-19 has underscored the importance of digital health: the digitalisation of healthcare would improve health systems’ resilience and service delivery, empower citizens, and bolster research and innovation. However, despite the many benefits of introducing technologies like artificial intelligence (AI) into the European healthcare sector, it also raises questions on its impact on the healthcare workers, the potential for new inequalities, privacy issues and public trust. Not to mention, broader questions on the ethics and social implications of AI. The EU’s Proposal for a Regulation Laying Down Harmonised Rules on Artificial Intelligence and the European Health Data Space address many of these concerns, but cannot combat them all alone. Without public trust, acceptance and understanding, AI’s full potential in healthcare will not be realised. Attention must be given to the communication of the benefits of AI in healthcare to the public, Europeans’ digital literacy, the upskilling of the EU health workforce, and the investment in AI technologies. Clear and convincing explanations regarding AI use in healthcare are required so that citizens understand their benefit and trust that their personal data will be handled safely. If the EU and its member states fail to build trust in the deployment of AI in healthcare, Europe will miss a chance to reap all the benefits that AI offers to its citizens and their health.

7. Estimating the UK's Return on Investment from an Ambitious Program to Incentivize New Antibiotics

Anti-microbial drugs form the backbone of modern medicine. Yet their lifespan is naturally limited; over time, use of these drugs selects for mutations that survive exposure those same drugs, driving “anti-microbial resistance,” or AMR. Already, drug-resistant infections kill an estimated 7,600 citizens of the United Kingdom (UK) every year. In the absence of sufficient research and development (R&D) investment for new antimicrobials, deaths from drug-resistant infections could increase dramatically in the coming decade. To address this growing crisis and solve market failures that prevent the development of new antibiotics, the UK has set up “subscription models” for purchasing antibiotics; this has been applied to two drugs thus far, and the model is under consideration for a broader rollout. In this note, we present the results of a modelling exercise to estimate the likely return on investment (ROI) from an expanded program to incentivize new antibiotics, assuming it is paired with complementary and proportionate efforts from the UK’s G7 partners. The results are necessarily imprecise due to several uncertain parameters, but nevertheless provide evidence of a very high expected ROI that is robust to different inputs and assumptions.

8. Does Europe need a Health Union?

Health outcomes in the European Union are good by international standards, even compared to other developed economies, and improved continuously before COVID-19. This reflects the alignment of the objectives of improving health and wellbeing with the overall socio-economic objective of prosperity, and suggests that a radical overhaul of EU health policy is not needed. However, the EU could benefit from closer integration in some areas and be more effective in delivering a high level of health protection. Action could be taken in the following areas: The European Commission’s November 2020 Health Union package to increase resilience to cross-border health threats is ambitious, in particular with the establishment of the Health Emergency Response Authority, which extends the scope of cooperation in health emergencies. The EU should pursue further the Health Union approach to address cross-border externalities and enhance health security. It could, for instance, do more to tackle anti-microbial resistance, or define minimum requirements for the resilience of health systems. For non-communicable diseases, the EU should tap the economies of scale of research and knowledge organised at EU level, and put in place systems for the surveillance of non-communicable diseases and consolidation of scientific knowledge. This could be achieved through an extension to non-communicable diseases of the European Centre for Disease Prevention and Control mandate. The ‘Health in all Policies’ principle is a key channel to deliver good health outcomes at EU level. EU scientific agencies provide health risk assessments in a number of areas, but the EU should better organise, coordinate and consolidate the scientific knowledge that underpins health-protection measures in sectoral legislation, and should more systematically apply better regulation rules to the health impacts of EU policies. The EU should support the digital transformation of health systems and set high targets for the European Health Data Space initiative, which is a critical infrastructure for the future of health research, regulation and policymaking. Several non-health EU policy objectives (cohesion policies, European Pillar of Social rights, economic governance) are linked to the performance of health systems. A common understanding on how to measure this performance would inform these policies in a consistent way. Moreover favourable health outcomes in the EU have not reduced health inequalities, which remain high between and within EU countries. Monitoring of health inequalities, including those related to access to and quality of healthcare, should be improved as an initial step.

Critics of narcology—as addiction medicine is called in Russia—decry it as being "backward," hopelessly behind contemporary global medical practices in relation to addiction and substance abuse, and assume that its practitioners lack both professionalism and expertise. On the basis of his research in a range of clinical institutions managing substance abuse in St. Petersburg, Eugene Raikhel increasingly came to understand that these assumptions and critiques obscured more than they revealed. Governing Habits is an ethnography of extraordinary sensitivity and awareness that shows how therapeutic practice and expertise is expressed in the highly specific, yet rapidly transforming milieu of hospitals, clinics, and rehabilitation centers in post Soviet Russia. Rather than interpreting narcology as a Soviet survival or a local clinical world on the wane in the face of globalizing evidence-based medicine, Raikhel examines the transformation of the medical management of alcoholism in Russia over the past twenty years. Raikhel's book is more than a story about the treatment of alcoholism. It is also a gripping analysis of the many cultural, institutional, political, and social transformations taking place in the post-Soviet world, particularly in Putin's Russia. Governing Habits will appeal to a wide range of readers, from medical anthropologists, clinicians, to scholars of post-Soviet Russia, to students of institutions and organizational change, to those interested in therapies and treatments of substance abuse, addiction, and alcoholism.

Abstract:
As the crisis phase of covid-19 recedes, there is a chance to improve international cooperation on global health – but also a danger that competing reform proposals will lead to inaction. The EU can best support reform of pandemic preparedness and response if it takes account of the concerns of different global powers. The union should combine a push for reform of and increased funding for the WHO with support for a new fund for health emergencies, overseen by a representative group of countries. The EU should promote a new global compact on health, matching countries’ commitment to surveillance and reporting of pathogens with support for stronger healthcare systems and greater equity in the allocation of countermeasures. The EU-Africa relationship offers a chance to pioneer such an approach, but the EU will need to go further in this than it has so far. The EU should promote African vaccine manufacturing, including by pressing European pharmaceutical companies to transfer knowledge and technology to Africa.

BRICS have been cast as a bloc with the potential to make significant changes in Global Health. The management of the Covid-19 pandemic has shown divisions in the bloc and the limits of its ability to formulate policies or even act upon previously agreed positions. This paper employs an examination of BRICS Health Ministerial declarations and an analysis of power in International Relations to reflect on BRICS’ Global Health diplomacy during the Covid-19 pandemic, covering the key questions of vaccine research and development, vaccine nationalism, and travel bans. It finds that multiple dimensions of power matter in Global Health leadership.

The war in Ukraine has imposed considerable physical, mental, and emotional damage on children. It is critical to provide mental health and psychosocial support (MPHSS), as well as to reinforce psychological and social infrastructure—in Ukraine and countries hosting refugees—to help them cope with the immediate trauma of war and promote resilience for future reconstruction efforts. Severe damage to civilian infrastructure has left many children without access to schools, healthcare resources, and support networks within care facilities, while those who have fled Ukraine risk falling victim to human trafficking and exploitation. Moreover, children are targets of the Kremlin’s efforts to erase the Ukrainian identity, brutally executed through oppressive policies such as forced school curriculum in the occupied territories and atrocious practice of ’filtration’, with children separated, abducted, and then put up for adoption in Russia. Inside and outside of the country, Ukraine’s youth are left extremely vulnerable to abuse, with the consequences to their mental health to last for generations—and to affect the nation’s post-war recovery—if not taken care of already today. Without developing proper coping mechanisms in children and adolescents now, the trauma imposed on the young generation will hinder rebuilding efforts and undermine societal institutions. It is thus imperative to support MHPSS initiatives and contribute to a resilient society in post-war Ukraine.

About one-quarter of U.S. adults report having a mental health diagnosis such as anxiety or depression or experiencing emotional distress. This is one of the highest rates among 11 high-income countries. While U.S. adults are among the most willing to seek professional help for emotional distress, they are among the most likely to report access or affordability issues. Emotional distress is associated with social and economic needs in all countries. Nearly half of U.S. adults who experience emotional distress report such worries, a higher share than seen in other countries. The United States has some of the worst mental health–related outcomes, including the highest suicide rate and second-highest drug-related death rate. The U.S. has a relatively low supply of mental health workers, particularly psychologists and psychiatrists. Just one-third of U.S. primary care practices have mental health professionals on their team, compared to more than 90 percent in the Netherlands and Sweden.

The health crisis has triggered a tectonic movement in the recomposition of health data governance and protection models around the world, while accelerating the investment of large digital companies in the field of e-health. The COVID-19 pandemic has revealed the shortcomings of pre-existing governance models in every region of the world and the need to move towards a model of public health crisis management "through health data". For Europe and for France in particular, the health crisis has revealed a long "technological innocence". Despite a strategic awakening, questions remain about the ability of Europeans to cooperate effectively and move towards a common digital space in health. In the United States, the pandemic has created tensions around the health data governance model – particularly on the articulation between the federal government and state authorities. Changes are announced by President Joe Biden, which tend towards a model of global regulation of personal data. In China, the pandemic has accelerated the transition to a model of surveillance capitalism which could lead in the short term to a new power struggle between the regime and the Chinese digital giants. Finally, the COVID-19 crisis has accelerated the rise of digital companies, which are now positioning themselves on the entire value chain of health data, from its raw collection via connected objects to its mass processing for insurance purposes. They now have all the levers in hand to economically value this massive data with the help of artificial intelligence.

19. China’s Public Procurement Protectionism and Europe’s Response: The Case of Medical Technology

This paper concerns China’s market for medical technologies and how the Chinese state is assisting its own companies to gain greater sales at the expense of producers from Europe and other advanced manufacturing economies. The medical technology sector captures a variety of products, services and solutions which are essential to the provision of healthcare to citizens. Examples range from fairly simple technologies such as sticking platers, to complex ones, such as coronary stents, orthopaedics and pacemakers. In the last decades, Europe’s exports of medical devices to China have grown robustly. On the back of vibrant innovation, firms from Europe and elsewhere have not just followed the growth of Chinese demand for medical devices – they have also increased their share of Chinese imports. Now, however, this market is at risk of being gradually closed off for European firms as China doubles down on various policies that advantage local firms, while ultimately harming innovation and Chinese patients. China has recently introduced a new element in its industrial policy for its medical technology sector – a policy that builds on its long-term ‘made-in-China’ ambition to have local firms taking up the lion’s share of the market for medical devices by 2030. Beijing and provincial governments are using many different policies to provide support to the domestic industry and are in effect pursuing a policy for import substitution. Accessing the Chinese market for medical devices has never been easy but it has become increasingly challenging in recent years. Direct financial support, tax benefits, R&D support, local content requirements, opaque approval systems and other forms of advantages to domestic MedTech producers are now becoming major sources for the behaviour and development of the Chinese medical device market. China’s Fourteenth Five-Year Plan and the country’s new economic model of ‘dual circulation’ reinforces this trend. The Chinese market is becoming less open. Added to that is centralised state procurement – a new public procurement policy in China that was launched in 2019 for medical devices which has led to a grossly distorted procurement market. While this policy has reduced the price of medical devices in a drastic way, it has also paved the way for Chinese firms to take up a larger market share. This procurement policy is gradually squeezing European firms out of the Chinese market. Competition in China is becoming less fair. These policies follow a pattern. In several sectors, Chinese firms have grown their domestic market share through industrial subsidies and policies that restrict market access for foreign firms – or make it more difficult for foreign firms to compete in China. From their domestic market, these firms have then expanded abroad and taken a more prominent role outside of China. Backed by the Chinese state, these firms can squeeze market opportunities even more for otherwise-competitive European producers. Now this development is happening in the MedTech sector. It is urgent for Europe to respond to China’s procurement protectionism. MedTech manufacturers in Europe are often small and medium-sized enterprises, and they already struggle with getting into the Chinese market. China’s new procurement policy has also had a significant impact on Chinese imports of medical devices: a 1.3 billion euro trade deficit for China in medical technology products in 2019 turned into a 5.2 billion euro surplus in 2020. In Africa, Asia and Latin America, competitive European manufacturers are now confronted with competition from Chinese MedTech companies that have the backing of the Chinese state. The competitive distortions in the Chinese market now spill over to the global market – making European MedTech manufacturers even more disadvantaged. Europe should consider a case at the World Trade Organisation (WTO) against China’s policies which reduce European market presence for medical devices. Once agreed, a new International Procurement Initiative might be a useful instrument to wield pressure against the Chinese government to agree to free and fair competition. Europe can also negotiate with China – for instance in the framework of the Comprehensive Agreement on Investment (CAI) or through the new initiative on trade and health that Europe proposed to the Ottawa Group in 2020. These responses should focus both on remedying China’s current policy for growing its domestic medical technology industry by distorting its market and setting out the core policies and rules that should guide the future market for medical technologies. Abstract:
The Taliban offensive in Afghanistan has deepened the humanitarian crisis in the country. Afghans are struggling now with forced displacement as well as a food and health crisis. The EU will increase its humanitarian aid, but its effectiveness will be limited by dangerous working conditions for NGOs. This requires an agreement with the Taliban to respect humanitarian law.
