1. Healthier Firms for a Stronger Recovery: Policies to Support Business and Jobs in Latin America and the Caribbean

Abstract:
This report—the product of a joint working group convened by the Center for Global Development and the Inter-American Development Bank—focuses on firms and labor markets in Latin America and the Caribbean during the COVID-19 crisis and the highly uncertain recovery phase now underway. The ongoing Russian war in Ukraine, volatility in international financial markets, and fears of global stagflation (low growth and high inflation) combine with the impacts of the pandemic to make the economic environment particularly challenging. Through a balance sheet analysis, the report describes how larger firms have navigated the crisis by cutting back on vari­able costs and investment. It also focuses on small and medi­um-size enterprises (SMEs), which appear to have suffered more in terms of closures and restrictions on credit access. The impacts on labor markets were unprecedented, with steep falls in employment and participation rates. A key question is whether the reallocation of resources, favor­ing firms in sectors that were hit less hard and able to take advantage of digital technologies, will persist. Key dangers are that informality is higher than ever, that productive resources remain trapped in small and less productive or ultimately unviable enterprises, and that firms are not rebuilding their capital stocks quickly. Drawing on an analysis of firms and labor markets, the report provides a set of recommendations for policymakers in the region and suggestions for international financial institutions to assist productive firms to invest, support the growth of new firms, and enhance labor market performance.

Abstract:
In 2006, when a CGD working group, led by Ruth Levine, Bill Savedoff, and Nancy Birdsall, published its report When Will We Ever Learn? Improving Lives Through Impact Evaluation, very few social programs benefitted from studies that could determine whether or not they actually make a difference. Since then, the world has seen tremendous progress in harnessing better evidence to inform public policy decision making, especially from impact evaluations of programs in low- and middle-income countries. But COVID-19 has put a spotlight on an unfinished agenda, underscoring the need for high-quality, timely, and context-specific evidence—for both effectiveness and political credibility of the response. The pandemic has demonstrated the cost in lives and livelihoods lost when policymakers make decisions based on incomplete or outdated evidence and data. Given the potential real-world benefits, why have decision makers within governments, aid agencies, multilateral organizations, and NGOs not yet fully harnessed the value of evidence—including from impact evaluations—for better public policies? Looking ahead, how can the development community renew momentum and broaden bases of support for impact evaluation and the wider evidence agenda? These questions were the focus of a CGD Working Group on New Evidence Tools for Policy Impact, which set out to understand why these social benefits continue to go unrealized and to chart out a renewed funding agenda for greater value in government policymaking.

This introduction tries to summarize the main arguments across all these contributions, noting points of consensus and ongoing debate.

Abstract:
Innovation—delivering new drugs, diagnostics, and devices—is a critical tool in the global fight against disease and premature death. Yet despite the potential for innovation to improve health around the world, the pharmaceutical industry’s investments in research and development (R&D) generally neglect diseases of the poor in favour of more lucrative high-income markets. Responding to this R&D gap, donor “push” investments have helped advance an innovation agenda to serve low- and middle-income countries. Though these investments have helped accelerate market entry of several important innovations, other donor-push products have fizzled upon market entry due to unaffordable or non-cost-effective pricing, disappointing efficacy, lack of political will, or lower-than-anticipated country demand. And with many large middle-income countries (MICs) poised to soon transition from donor aid, the sustainability of the current donor-led model is in question. Tuberculosis (TB), an infectious disease primarily affecting the poor and vulnerable, ranks among the top 10 global causes of death. Current TB treatment cycles are long and toxic, causing some patients to discontinue treatment, develop acquired drug resistance, and risk spreading a drug-resistant pathogen to others. Drug-resistant strains are more difficult to treat, traditionally requiring long-duration toxic regimens and high-cost hospitalization (though recent innovations offer a shorter, more tolerable, and more affordable treatment). Despite years of global investment in TB control, modelling suggests that global goals for TB cannot be achieved without major technological breakthroughs. One particularly desirable innovation would be a short-course universal drug regimen (UDR)—equally capable of treating drug-sensitive and drug-resistant strains, with a two-month or shorter treatment duration. Donors, particularly the Bill & Melinda Gates Foundation (BMGF), have funded substantial early-stage R&D to source new treatment compounds that could contribute to a UDR, but substantial additional investments in late-stage trials would be required to bring such a UDR to market.

8. Focusing on Fragility: The Future of US Assistance to Fragile States

Global development is increasingly intertwined with state fragility. Poverty is becoming concentrated in fragile states, and conflict, violent extremism, and environmental stresses can both stem from and be ex- acerbated by fragility. As a result, many donors—including the United States—are rethinking how their engagement can better help countries address the underlying causes of fragility, build peace and stability, and cope with complex risks. The United States is the top provider of official development assistance to fragile states, but much of this aid has not focused on reducing fragility. Even where confronting fragility has been a central objec- tive of US development assistance, the track record of success has been mixed, at best. New efforts are emerging to change this story. Reflecting on lessons of the past, the United States government has been developing new policy frameworks, initiatives, and proposed reforms that seek to address shortcomings in how the US government engages in fragile states. To contribute to this conversation, the Center for Global Development convened a working group of more than 20 experts, including former officials from the State Department, the United States Agency for International Development (USAID), the Department of Defense (DOD), and the US intelligence community, along with noted academics and policy experts to identify specific ideas for how the US government can more effectively use its develop- ment assistance—in conjunction with diplomatic and security assistance tools—in fragile states.

9. The Dilemma of the African Development Bank: Does Governance Matter for the Long-Run Financing of the MDBs?

Does governance matter for the long-run financing of the multilateral development banks? The structure of governance of the legacy MDBs (the World Bank and the four major regional development banks founded in the twentieth century) ideally should minimize any tradeoff between the confidence of creditor shareholding countries, on which an MDB’s own financing depends, and the sense of ownership, legitimacy, and trust of borrowing countries, on which the MDB’s effectiveness in supporting development in those borrowing countries depends. Among the five legacy MDBs, the African Development Bank stands out as the one where the governance arrangements, including the distribution of shares and votes between borrowers and nonborrowers, most favors borrowers. Indicators of the AfDB’s relative financial strength (a measure of creditworthiness based on sovereign members’ vote shares, and a measure of the capacity of each bank’s members to engage in collective action or cooperation in raising financing) indicate that its current governance is likely to make it less competitive than its sister MDBs in sustaining creditor (or “donor”) confidence in its operations over the long run, and thus in raising substantial capital and concessional resources. The governance problem is most obvious in the case of the African Development Bank’s African Development Fund, which today has only about 15 percent of the resources the World Bank has for Africa. The creditors of the AfDB have sufficient control to ensure the Bank’s financial soundness (and AAA rating), but a collective action constraint in pushing for reforms in the Bank’s operations. The paper concludes with ideas for long-run reform of governance at the African Development Bank, modeled more closely on the governance of the Inter-American Development Bank.
