COVID-19 has underscored the importance of digital health: the digitalisation of healthcare would improve health systems’ resilience and service delivery, empower citizens, and bolster research and innovation. However, despite the many benefits of introducing technologies like artificial intelligence (AI) into the European healthcare sector, it also raises questions on its impact on the healthcare workers, the potential for new inequalities, privacy issues and public trust. Not to mention, broader questions on the ethics and social implications of AI. The EU’s Proposal for a Regulation Laying Down Harmonised Rules on Artificial Intelligence and the European Health Data Space address many of these concerns, but cannot combat them all alone. Without public trust, acceptance and understanding, AI’s full potential in healthcare will not be realised. Attention must be given to the communication of the benefits of AI in healthcare to the public, Europeans’ digital literacy, the upskilling of the EU health workforce, and the investment in AI technologies. Clear and convincing explanations regarding AI use in healthcare are required so that citizens understand their benefit and trust that their personal data will be handled safely. If the EU and its member states fail to build trust in the deployment of AI in healthcare, Europe will miss a chance to reap all the benefits that AI offers to its citizens and their health.

2. Estimating the UK's Return on Investment from an Ambitious Program to Incentivize New Antibiotics

Anti-microbial drugs form the backbone of modern medicine. Yet their lifespan is naturally limited; over time, use of these drugs selects for mutations that survive exposure those same drugs, driving “anti-microbial resistance,” or AMR. Already, drug-resistant infections kill an estimated 7,600 citizens of the United Kingdom (UK) every year. In the absence of sufficient research and development (R&D) investment for new antimicrobials, deaths from drug-resistant infections could increase dramatically in the coming decade. To address this growing crisis and solve market failures that prevent the development of new antibiotics, the UK has set up “subscription models” for purchasing antibiotics; this has been applied to two drugs thus far, and the model is under consideration for a broader rollout. In this note, we present the results of a modelling exercise to estimate the likely return on investment (ROI) from an expanded program to incentivize new antibiotics, assuming it is paired with complementary and proportionate efforts from the UK’s G7 partners. The results are necessarily imprecise due to several uncertain parameters, but nevertheless provide evidence of a very high expected ROI that is robust to different inputs and assumptions.

3. Does Europe need a Health Union?

Health outcomes in the European Union are good by international standards, even compared to other developed economies, and improved continuously before COVID-19. This reflects the alignment of the objectives of improving health and wellbeing with the overall socio-economic objective of prosperity, and suggests that a radical overhaul of EU health policy is not needed. However, the EU could benefit from closer integration in some areas and be more effective in delivering a high level of health protection. Action could be taken in the following areas: The European Commission’s November 2020 Health Union package to increase resilience to cross-border health threats is ambitious, in particular with the establishment of the Health Emergency Response Authority, which extends the scope of cooperation in health emergencies. The EU should pursue further the Health Union approach to address cross-border externalities and enhance health security. It could, for instance, do more to tackle anti-microbial resistance, or define minimum requirements for the resilience of health systems. For non-communicable diseases, the EU should tap the economies of scale of research and knowledge organised at EU level, and put in place systems for the surveillance of non-communicable diseases and consolidation of scientific knowledge. This could be achieved through an extension to non-communicable diseases of the European Centre for Disease Prevention and Control mandate. The ‘Health in all Policies’ principle is a key channel to deliver good health outcomes at EU level. EU scientific agencies provide health risk assessments in a number of areas, but the EU should better organise, coordinate and consolidate the scientific knowledge that underpins health-protection measures in sectoral legislation, and should more systematically apply better regulation rules to the health impacts of EU policies. The EU should support the digital transformation of health systems and set high targets for the European Health Data Space initiative, which is a critical infrastructure for the future of health research, regulation and policymaking. Several non-health EU policy objectives (cohesion policies, European Pillar of Social rights, economic governance) are linked to the performance of health systems. A common understanding on how to measure this performance would inform these policies in a consistent way. Moreover favourable health outcomes in the EU have not reduced health inequalities, which remain high between and within EU countries. Monitoring of health inequalities, including those related to access to and quality of healthcare, should be improved as an initial step.

Abstract:
As the crisis phase of covid-19 recedes, there is a chance to improve international cooperation on global health – but also a danger that competing reform proposals will lead to inaction. The EU can best support reform of pandemic preparedness and response if it takes account of the concerns of different global powers. The union should combine a push for reform of and increased funding for the WHO with support for a new fund for health emergencies, overseen by a representative group of countries. The EU should promote a new global compact on health, matching countries’ commitment to surveillance and reporting of pathogens with support for stronger healthcare systems and greater equity in the allocation of countermeasures. The EU-Africa relationship offers a chance to pioneer such an approach, but the EU will need to go further in this than it has so far. The EU should promote African vaccine manufacturing, including by pressing European pharmaceutical companies to transfer knowledge and technology to Africa.

Abstract:
The war in Ukraine has imposed considerable physical, mental, and emotional damage on children. It is critical to provide mental health and psychosocial support (MPHSS), as well as to reinforce psychological and social infrastructure—in Ukraine and countries hosting refugees—to help them cope with the immediate trauma of war and promote resilience for future reconstruction efforts. Severe damage to civilian infrastructure has left many children without access to schools, healthcare resources, and support networks within care facilities, while those who have fled Ukraine risk falling victim to human trafficking and exploitation. Moreover, children are targets of the Kremlin’s efforts to erase the Ukrainian identity, brutally executed through oppressive policies such as forced school curriculum in the occupied territories and atrocious practice of ’filtration’, with children separated, abducted, and then put up for adoption in Russia. Inside and outside of the country, Ukraine’s youth are left extremely vulnerable to abuse, with the consequences to their mental health to last for generations—and to affect the nation’s post-war recovery—if not taken care of already today. Without developing proper coping mechanisms in children and adolescents now, the trauma imposed on the young generation will hinder rebuilding efforts and undermine societal institutions. It is thus imperative to support MHPSS initiatives and contribute to a resilient society in post-war Ukraine.

7. Combating Unsafe Products: How to Improve Europe’s Safety Gate Alerts

Abstract:
The European Union has built a one-stop-shop for its member state regulators to post product safety notifications – Safety Gate (European Commission 2021d). Constructed on top of the Rapid Alert System for Dangerous Non-Food Products, or RAPEX, the Safety Gate web portal is designed to make public the “quick exchange of information” between 31 European countries and the European Commission “about measures taken against dangerous non-food products.” While Safety Gate represents a significant achievement, our research revealed areas for improvement to increase its utility for manufacturers, marketplaces and consumers. Many product notifications published on the website lack details required to facilitate speedy removals and recalls. The study graded eight essential criteria for a total of 918 Safety Gate notifications published over eight months in 2020. The average notification score was a respectable 70 out of 100, but over 98% of the notifications omitted at least one key criterion. Only 14 notifications included all the information to enable efficient and accurate product identification.

During the Covid-19 pandemic, Europe has benefitted strongly from being an open economy that can access goods and services from other parts of the world. Paradoxically, some politicians in Europe think that dependence on foreign supplies reduced the resilience of our economy – and argue that Europe now should wean itself off its dependence on other economies. In this Policy Brief, it is argued that self-sufficiency or less economic openness is a dangerous direction of policy. It would make Europe less resilient and less capable of responding to the next emergency. It is key that people, firms and governments can get supplies from other parts of the world. It is diversification, not concentration of production, that will make Europe more resilient when the next emergency hit. We don’t know where the next crisis will come from. Nature will throw nasty surprises at us, and we will make stupid mistakes, some of which will have devastating consequences. What we do know, though, is that we stand a better chance to fight the next emergency if we get richer and improve our technology. The best policy for resilience is one that encourages specialisation and innovation – and, when the emergency hit, allow for people to improvise in search for solutions. For that to happen, we need openness to goods, services and technology from abroad.

9. China’s Public Procurement Protectionism and Europe’s Response: The Case of Medical Technology

Abstract:
This paper concerns China’s market for medical technologies and how the Chinese state is assisting its own companies to gain greater sales at the expense of producers from Europe and other advanced manufacturing economies. The medical technology sector captures a variety of products, services and solutions which are essential to the provision of healthcare to citizens. Examples range from fairly simple technologies such as sticking platers, to complex ones, such as coronary stents, orthopaedics and pacemakers. In the last decades, Europe’s exports of medical devices to China have grown robustly. On the back of vibrant innovation, firms from Europe and elsewhere have not just followed the growth of Chinese demand for medical devices – they have also increased their share of Chinese imports. Now, however, this market is at risk of being gradually closed off for European firms as China doubles down on various policies that advantage local firms, while ultimately harming innovation and Chinese patients. China has recently introduced a new element in its industrial policy for its medical technology sector – a policy that builds on its long-term ‘made-in-China’ ambition to have local firms taking up the lion’s share of the market for medical devices by 2030. Beijing and provincial governments are using many different policies to provide support to the domestic industry and are in effect pursuing a policy for import substitution. Accessing the Chinese market for medical devices has never been easy but it has become increasingly challenging in recent years. Direct financial support, tax benefits, R&D support, local content requirements, opaque approval systems and other forms of advantages to domestic MedTech producers are now becoming major sources for the behaviour and development of the Chinese medical device market. China’s Fourteenth Five-Year Plan and the country’s new economic model of ‘dual circulation’ reinforces this trend. The Chinese market is becoming less open. Added to that is centralised state procurement – a new public procurement policy in China that was launched in 2019 for medical devices which has led to a grossly distorted procurement market. While this policy has reduced the price of medical devices in a drastic way, it has also paved the way for Chinese firms to take up a larger market share. This procurement policy is gradually squeezing European firms out of the Chinese market. Competition in China is becoming less fair. These policies follow a pattern. In several sectors, Chinese firms have grown their domestic market share through industrial subsidies and policies that restrict market access for foreign firms – or make it more difficult for foreign firms to compete in China. From their domestic market, these firms have then expanded abroad and taken a more prominent role outside of China. Backed by the Chinese state, these firms can squeeze market opportunities even more for otherwise-competitive European producers. Now this development is happening in the MedTech sector. It is urgent for Europe to respond to China’s procurement protectionism. MedTech manufacturers in Europe are often small and medium-sized enterprises, and they already struggle with getting into the Chinese market. China’s new procurement policy has also had a significant impact on Chinese imports of medical devices: a 1.3 billion euro trade deficit for China in medical technology products in 2019 turned into a 5.2 billion euro surplus in 2020. In Africa, Asia and Latin America, competitive European manufacturers are now confronted with competition from Chinese MedTech companies that have the backing of the Chinese state. The competitive distortions in the Chinese market now spill over to the global market – making European MedTech manufacturers even more disadvantaged. Europe should consider a case at the World Trade Organisation (WTO) against China’s policies which reduce European market presence for medical devices. Once agreed, a new International Procurement Initiative might be a useful instrument to wield pressure against the Chinese government to agree to free and fair competition. Europe can also negotiate with China – for instance in the framework of the Comprehensive Agreement on Investment (CAI) or through the new initiative on trade and health that Europe proposed to the Ottawa Group in 2020. These responses should focus both on remedying China’s current policy for growing its domestic medical technology industry by distorting its market and setting out the core policies and rules that should guide the future market for medical technologies. The COVID-19 crisis has prompted the European Union (EU) to rethink its health policy, or rather those of its policies that influence the health policies of member states, as those largely comprise a national competence, and sometimes a subnational one. During the pandemic, EU institutions and EU member states identified issues where more EU coordination was desirable, for instance with regard to stockpiling and joint purchasing of medical products. Much is still unclear, however, about how a broadly supported revised EU health policy should look, particularly as this has traditionally been a field where EU citizens and EU member states saw little added value in the EU becoming involved. A newly proposed EU4Health programme saw a setback right at its inception, with its proposed funding being cut drastically by the European Council, even though EU health expenditure will continue to rise. This policy brief explores the future of EU health policy after the COVID-19 pandemic changed conventional thinking.

Coordinated Covid-19 crisis management among EU member states took weeks to develop. The initial response was slow, at times contradictory, frequently displaying nation-state unilateralism and obviously divisive. An unprecedented architecture of fiscal and monetary partnership is now building in Europe. The ECB is taking the lead in this construction process, with national governments asserting their political prerogatives while the Commission in Brussels is careful not to overstep the national sovereignty of its member states. As the crisis response to Covid-19 has highlighted manifest fault lines among EU member states, these divisions could morph into bigger controversies during uncoordinated exit strategies. Once the cessation of economic activity can start to unwind, the debate across Europe will accelerate over budget rules, further relief programmes and the demand for debt forbearance.

As the Juncker Presidency hands over its mandate baton to Ursula von der Leyen, all eyes are on the energised new President and her ambitions bestowed upon her cabinet. Despite some progress, more efforts to improve Europe’s health policy is still needed if it is to tackle unprecedented challenges like demographic changes, environmental degradation and the rapidly changing world of work. Newly-appointed Health Commissioner Stella Kyriakides has been entrusted with a considerable portfolio, indicating an arduous yet promising five years ahead of her. In this Policy Brief, policy analyst Simona Guagliardo argues that von der Leyen’s agenda for a “Union that strives for more” offers a unique opportunity to build a strong case for placing health and well-being at the centre of her policy triad: economic growth fuelled by technological innovation and environmental protection. The EU and its member states must recognise the centrality of people’s health and well-being vis-à-vis economic growth; ensure that health is a constant factor in all policymaking; and deliver on their promises of social fairness, equality and inclusion.

he COVID-19 pandemic has exposed the unpreparedness of European health systems to absorb a health crisis of this magnitude. In order to fix these existing structural weaknesses and effectively deal with the health challenges brought on by the current pandemic, we need to build a ‘Europe of Health’; an EU public health policy that works across borders and lives up to its citizens’ expectations. Following on from the Franco-German initiative for European recovery from the ongoing COVID-19 crisis, Claire Dhéret and Simona Guagliardo outline a twofold strategy to foster ‘EU health sovereignty’ and reinforce the EU’s international leadership in health at a time when multilateralism is seriously jeopardised: Foster EU health crisis management capabilities by empowering existing structures, prepare a European stockpile of strategic medical products and equipment, and make better use of the Single Market in the area of health. Strengthen the resilience of national health systems by promoting modern, innovative and patient-centred care, and encourage the creation and use of a common, EU-wide evaluation framework for health care.

16. Sovereign Europe, dangerous world: Five agendas to protect Europe’s capacity to act

In the last year, Europe has begun to recognise the need to defend its sovereignty in a threatening world. Covid-19 has revealed and exacerbated many of Europe’s existing vulnerabilities; the European Union and its member states remain unable to act autonomously in key areas of national life. Based on ECFR research, we propose five sovereignty agendas in health, economic, digital, climate change, and traditional security, all designed to promote a more sovereign Europe on that issues that matter most to Europeans. Europe must not relinquish its rules-based approach or lapse into protectionism. But, to protect the open, multilateral order they so cherish, Europeans need to promote new rules permitting them to take action against countries that undermine the international system. The EU’s €750 billion pandemic recovery fund offers the chance to underwrite this ambition – but, ultimately, Europeans need to master the art of acting as a geopolitical force in the world.

As Health Secretary Matt Hancock returns to his role as part of Boris Johnson’s premiership, he has an opportunity to make good on the UK’s renewed confidence and ambition by drawing on what the new prime minister calls the “best healthcare [system]” to drive improvements in health globally. The UK’s Department of Health and Social Care controls its biggest-ever official development assistance (ODA) budget, doubling between 2017 and 2018 to just under £200m, or 1.3 percent of the country’s aid allocation. This places the UK’s secretary of state for health in a unique position to truly make a difference in countries’ journeys towards universal healthcare coverage (UHC), whilst also defending (and making a case for more of) the ODA money his department has been allocated, even to development aid’s harshest critics. While private provision of goods often yields the efficient outcome, there are a number of goods that are not efficiently provided in the private market. Here, we outline two such situations: investments in child care and education, and insurance against risks created by business cycles, poor health, and old age. Because private markets work poorly for these goods, and the costs of market failure are large, standard economic reasoning implies a significant role for government provision. The reduction in economic insecurity that this would bring could help to improve political stability as well, by reducing the stakes that people perceive in discussions of trade, immigration, technological change, and countercyclical policy (Inglehart and Norris, 2016). Many observers (e.g, Hacker, 2018) have pointed to economic anxiety as a potential contributor to populist reactions in the U.S. and many European countries; a public sector that acts to reduce the risk that households face could ameliorate this, generating political spillovers and improving the state of the country more broadly.

The strong and uncompromising positions of countries opposed to the inclusion of foreign airlines in the EU's emissions trading system (ETS) are more related to a realist game of politics rather than to the design details of the policy instrument. The political and legal arguments against the European Commission's proposal to amend the EU ETS vis à vis aviation emissions are unconvincing. Europe should also insist on its own sovereign rights-such as the right to regulate international aviation in its own airspace-and consider ways of manifesting more assertiveness in the future in order not to create a precedent with the retreat in the Aviation Directive case. Otherwise, the EU ma y become vulnerable to pressure in other areas of regulation with extraterritorial implications, and the EU's credibility when faced with strong and coordinated external influences might be undermined.

ZeroWIN (Towards Zero Waste in Industrial Networks) is a five-year project (2009-2014) under the European Commission's Seventh Research Framework Programme. The ZeroWIN project has developed effective strategies for waste prevention through industrial networks. Ten industrial case studies in the automotive, construction, electronics and photovoltaic industries form the core of the project and exchange energy, water and materials in such a way that waste from one industry becomes raw material for another. This brief suggests what can be done to advance the implementation of industrial networks in practice, based on first outcomes of the ZeroWIN project. Abstract:
The purpose of this policy brief is to provide an assessment about the risk of uncontrolled use of pyrotechnic means in Kosovo, suggesting taking the necessary measures towards reducing the eventual consequences which may be caused by this phenomenon. Certainly, this argument is based on the perspective of the security sector, as a result of the real implications on safety and public order and environmental security. Although the legal basis exists, state institutions have not created the basic conditions to prevent possible incidents. In this regard, there were not foreseen and undertaken awareness campaigns on the use of pyrotechnic means and it's side effects - for human health, property and the environment - plants (air and water pollution) and animals; there have not been taken the necessary measures for the qualification of the people who trade these tools, sales without permission were not prohibited, the consumers are not being protected.

Access to medicines poses a critical challenge in developing countries, largely because prices are high, and new or adapted medicines and vaccines to address diseases of the developing world are lacking. More than 5 million people in low and middle income countries still lack access to the anti-retroviral medicines needed to treat HIV and AIDS. Non-communicable diseases (NCDs) have unleashed a new epidemic of suffering across the developing world. Pandemics are a serious threat in rich and poor countries alike, but while rich countries can stockpile medicines, these are often unaffordable for poor countries. Most people in developing countries pay for medicines out-of-pocket, so even a slight price increase can mean that life-saving medicines are unaffordable.

Developing-country governments desperately need more long-term and predictable aid, given through their budgets, to finance the expansion of health care, education, and other vital social services. The European Commission (EC) is one of the biggest donors providing this kind of essential budget support, and has innovative plans to further improve and increase this aid. European Union (EU) member states must support these ambitious plans. The EC in turn must do more to improve on this good start, delinking this aid from harmful International Monetary Fund (IMF) prescriptions, putting an end to unnecessary bureaucratic delays, and doing more to make its aid accountable to citizens in poor countries.

Providing all beneficial care to those who need it is rapidly becoming unaffordable, even for a nation as rich as the United States. The highly decentralized U.S. payment system is unique in its lack of effective levers for limiting health care spending, and managed care has largely been ineffective. A different solution, considered extreme by many in the United States, is rationing. The need to ration healthcare has long been a reality in the United Kingdom where healthcare spending must be covered by an annual budget accounting for only 7.6 percent of GDP—about half the U.S. share. These decisions are perhaps most difficult in regards to treatment of conditions that are literally matters of life or death, such as coronary artery disease. This brief examines reasons for the differences in treatment and outcomes in the United States and Britain, and discusses the difficulty of rationing care in the United States, where a unique payment system now uses income from those with health insurance to cover the medical costs of the uninsured.
