1. Health of nations: How Europe can fight future pandemics

Abstract:
As the crisis phase of covid-19 recedes, there is a chance to improve international cooperation on global health – but also a danger that competing reform proposals will lead to inaction. The EU can best support reform of pandemic preparedness and response if it takes account of the concerns of different global powers. The union should combine a push for reform of and increased funding for the WHO with support for a new fund for health emergencies, overseen by a representative group of countries. The EU should promote a new global compact on health, matching countries’ commitment to surveillance and reporting of pathogens with support for stronger healthcare systems and greater equity in the allocation of countermeasures. The EU-Africa relationship offers a chance to pioneer such an approach, but the EU will need to go further in this than it has so far. The EU should promote African vaccine manufacturing, including by pressing European pharmaceutical companies to transfer knowledge and technology to Africa.

2. China’s Public Procurement Protectionism and Europe’s Response: The Case of Medical Technology

Abstract:
This paper concerns China’s market for medical technologies and how the Chinese state is assisting its own companies to gain greater sales at the expense of producers from Europe and other advanced manufacturing economies. The medical technology sector captures a variety of products, services and solutions which are essential to the provision of healthcare to citizens. Examples range from fairly simple technologies such as sticking platers, to complex ones, such as coronary stents, orthopaedics and pacemakers. In the last decades, Europe’s exports of medical devices to China have grown robustly. On the back of vibrant innovation, firms from Europe and elsewhere have not just followed the growth of Chinese demand for medical devices – they have also increased their share of Chinese imports. Now, however, this market is at risk of being gradually closed off for European firms as China doubles down on various policies that advantage local firms, while ultimately harming innovation and Chinese patients. China has recently introduced a new element in its industrial policy for its medical technology sector – a policy that builds on its long-term ‘made-in-China’ ambition to have local firms taking up the lion’s share of the market for medical devices by 2030. Beijing and provincial governments are using many different policies to provide support to the domestic industry and are in effect pursuing a policy for import substitution. Accessing the Chinese market for medical devices has never been easy but it has become increasingly challenging in recent years. Direct financial support, tax benefits, R&D support, local content requirements, opaque approval systems and other forms of advantages to domestic MedTech producers are now becoming major sources for the behaviour and development of the Chinese medical device market. China’s Fourteenth Five-Year Plan and the country’s new economic model of ‘dual circulation’ reinforces this trend. The Chinese market is becoming less open. Added to that is centralised state procurement – a new public procurement policy in China that was launched in 2019 for medical devices which has led to a grossly distorted procurement market. While this policy has reduced the price of medical devices in a drastic way, it has also paved the way for Chinese firms to take up a larger market share. This procurement policy is gradually squeezing European firms out of the Chinese market. Competition in China is becoming less fair. These policies follow a pattern. In several sectors, Chinese firms have grown their domestic market share through industrial subsidies and policies that restrict market access for foreign firms – or make it more difficult for foreign firms to compete in China. From their domestic market, these firms have then expanded abroad and taken a more prominent role outside of China. Backed by the Chinese state, these firms can squeeze market opportunities even more for otherwise-competitive European producers. Now this development is happening in the MedTech sector. It is urgent for Europe to respond to China’s procurement protectionism. MedTech manufacturers in Europe are often small and medium-sized enterprises, and they already struggle with getting into the Chinese market. China’s new procurement policy has also had a significant impact on Chinese imports of medical devices: a 1.3 billion euro trade deficit for China in medical technology products in 2019 turned into a 5.2 billion euro surplus in 2020. In Africa, Asia and Latin America, competitive European manufacturers are now confronted with competition from Chinese MedTech companies that have the backing of the Chinese state. The competitive distortions in the Chinese market now spill over to the global market – making European MedTech manufacturers even more disadvantaged. Europe should consider a case at the World Trade Organisation (WTO) against China’s policies which reduce European market presence for medical devices. Once agreed, a new International Procurement Initiative might be a useful instrument to wield pressure against the Chinese government to agree to free and fair competition. Europe can also negotiate with China – for instance in the framework of the Comprehensive Agreement on Investment (CAI) or through the new initiative on trade and health that Europe proposed to the Ottawa Group in 2020. These responses should focus both on remedying China’s current policy for growing its domestic medical technology industry by distorting its market and setting out the core policies and rules that should guide the future market for medical technologies. Abstract:
Nearly 18 months after word of a deadly new virus began leaking out of Wuhan, China, the Chinese government’s response remains fundamentally hostile to international cooperation and transparency. Despite hundreds of offers of assistance, polite diplomatic entreaties, and demands for access to data by governments and health authorities across the globe, the world still knows far too little about COVID-19’s origin. As in a Dali painting, the clocks have melted but time has not stood still. China’s initial silencing and censoring of its doctors and scientists, followed by misinformation about COVID-19’s dangers—especially denials concerning the virus’s ability to be spread human-to-human, invisibly and asymptomatically—helped cost the world trillions of dollars and millions of lives. Whether one believes COVID-19 originated in a zoonotic host, a bat cave, a frozen food shipment, or a Wuhan lab’s dangerous “dual-use” research supporting undeclared bioweapons programs, the world needs answers from the Chinese Communist Party. These are answers Beijing won’t provide unless it faces a high price for refusing. For the good of public health and international security, the Biden administration and the Congress can unite in a coordinated, long-term response.

Abstract:
The Biden administration’s Interim National Security Strategic Guidance reflects an evolution in US strategic thinking and policy priorities. The document shows how the administration intends to shift away from America Alone and towards America and its Allies. But the optimism and confidence the US expressed in 2015 has been replaced by deep concern over a range of strategic trends. The US will now prioritise strategic competition with China, a new approach to trade, the rise of technology, the defence of democracy, the urgent climate and health crises, and efforts to avoid ‘forever wars’ in the Middle East.

The clergy’s ambitions for global Shia revolution made the city of Qom uniquely vulnerable to the disease, and their resistance to modern medical science weakened the state’s ability to combat its spread. On February 19, two days before the Iranian government officially announced the arrival of coronavirus, an infected businessman who had recently returned from China to Qom passed away. The location and timing of his death illustrate how the Shia holy city and the religious leaders and institutions who call it home have played an outsize role in the disease’s disproportionately rapid spread inside Iran compared to other countries. How did this situation come to pass, and what does it say about the current state of the clerical establishment, its relationship with the regime, and its alienation from large swaths of Iranian society? The spread of the novel Corona Virus COVID-19 from the Chinese province of Hebei across the world has resulted in a global pandemic of catastrophic proportions. Certain countries have been affected more severely than others, and there have been glaring disparities in how national governments have responded to the outbreak. In addition to the global death toll of 400,000 (and counting), the industrial and financial disruption has been severe, with the Asian Development Bank estimating the loss to the global economy at between USD$ 5.8-8.8 trillion.1 To overcome the current crisis, and work toward a vaccine, global solidarity, including cooperation through multilateral organizations like the World Heath Organization (WHO), is desperately needed.

The fight against COVID-19 has been the greatest challenge the world has faced since the middle of last century. As countries have fought to control the disease, they have closed borders, quarantined their citizens, and shut down economies almost entirely. The ramifications will reverberate for years, if not decades, to come. In April 2020, the Lowy Institute published a digital feature in which twelve Institute experts examined the ways in which the COVID crisis would affect Australia, the region and the world. In this new feature, Lowy Institute experts provide policy recommendations for Australia to address issues that are critical to our nation’s — and the world’s — successful emergence from the pandemic. Countries have turned inwards in an attempt to fend off the threat of an infection that is oblivious to borders. Some have seen globalisation as the cause of the crisis, and have focused on solving problems without recourse to the international institutions of global security and prosperity, including the United Nations, the World Health Organization, and the G20. Yet global problems require international solutions. As the world emerges from the crisis, cooperation between nations will be more important than ever. Nation states cannot revive their economies purely through national solutions. They cannot address global threats, including the possibility of further pandemics, alone. Australia’s achievements in managing the COVID crisis have been exemplary. It has handled the health and economic emergency with great competence. But this is just the beginning of our crisis recovery. The challenges in our region, and the global problems that existed before COVID, have only been exacerbated by the pandemic. Australia has already done much to address the domestic economic and health issues from the COVID crisis. But to shape a prosperous and secure future, it will also need to work in cooperation with other nations, large and small, allies and partners, on a much broader array of international issues ranging from the economic disruption across the region, pressure from China on trade, and development challenges in the Pacific, to increasingly competitive relations between the United States and China, the weakening of the World Health Organization, and the declining utility of the G20.

8. Toxic Legacy: Hunger, Oppression, Migration, and Health in the Democratic People's Republic of Korea

Abstract:
During the 1990s, economic mismanagement, political oppression, natural disaster, and loss of external subsidies after the end of communism led to a calamitous decrease in food production in the Democratic People's Republic of Korea (DPRK). The public health infrastructure, including water and sanitation systems, drug distribution and supply chains, and local clinics and hospitals, also deteriorated. At least half a million people died of starvation and millions more suffered acute or chronic malnutrition. Malnutrition increased vulnerability to disease at a time when the health system was incapable of effective response. Fifeen years later, neither health nor the food systems have recovered as the economy persistently stagnates. Health continues to be a low priority for the government. The availability of food is insufficient to meet population needs, hospitals and clinics are significantly ill-equipped, the medical workforce lacks appropriate training, and corruption in drug distribution is pervasive. Malnutrition and anemia, as well as diseases associated with poor sanitation, remain widespread. Over the last few years, DPRK has begun to accept international assistance to address health system needs, most notably to vaccinate children. Although these initiatives address some infrastructure needs, the continued centralized control of health and the lack of open discussion about key issues renders the possibility of reforms sufficient to meet the health needs of the people of North Korea dim. During the economic crisis, tens of thousands of North Koreans migrated to China despite harsh measures imposed by both governments to restrict border crossing and a refusal by China to give legal status to the migrants. To a limited extent, migration ameliorated the health impact of the crisis by stimulating illicit cross-border trade and informal markets that increased some North Koreans' access to food. Even after a disastrous effort by the DPRK government to shut the markets down in 2009, they are re-emerging. China's encouragement of these markets, along with regularizing the status of migrants in China, could advance its own economic interests as well as contributing to improving the health of North Koreans.

Improving adolescent girls' health and wellbeing is critical to achieving virtually all international development goals, from reducing infant and child deaths to stimulating economic growth and encouraging environmental sustainability. Governments and donors seem to recognize this, but they have yet to take the specific actions needed to genuinely invest in adolescent girls' health and, thereby, the health and wellbeing of generations to come.
